Two Patients With Severe Insulin Resistance

Biological evidence of hyperinsulinaemia is important for the diagnosis of Leprechaunism. In such cases, growth hormone and IGF-1 levels may be low in a patient with Donohue Syndrome, due to feedback from very high insulin levels sending signals through the hypothalamic and pituitary IGF receptors. The leptin concentrations also may be low due to poor adipose tissue development. Below are two cases of children with severe insulin resistance, Case 1 representing an infant with Leprechaunism.
